The Individual Support Plan and current waiver assignment tell us immediately whether we can staff the authorized hours. Our referral pathway sets out the full process.

The Merrimac Trail address is our office, not a place services are delivered from. In-Home Support and Independent Living happen where the individual already lives, so you do not need to travel to us — see our service areas for the communities we currently cover. If you are outside them, ask anyway and we will give you a straight answer.
