Information for families, Support Coordinators, and case managers referring someone to CoreCare.
A family member, Support Coordinator, case manager, or the individual themselves — anyone can start the conversation.
Call CoreCare directly or send a message through our contact form with a brief description of the individual's needs.
Our team reaches out to discuss the individual's needs and what support could look like — a real conversation, not a form disappearing into a queue.
We work with you to build a care plan that fits, together.
You do not need paperwork in hand to get in touch — a phone call is enough to start. It does speed things up if you can tell us the individual's general support needs, the area they live in, and roughly the schedule that would help.
If you are a Support Coordinator or case manager, the Individual Support Plan and the current waiver assignment are the most useful things to share, because they tell us straight away whether we can staff the authorized hours.
Not sure how the funding side works? Our guide to Virginia's DD Waivers covers it in plain language, and our service areas page sets out where we can currently take referrals.
We review every referral against two questions: whether the support being asked for is something we actually provide, and whether we can staff it reliably at that address. You get both answers either way.
If we can take it, the next step is a conversation with the individual and the people closest to them, so that whoever will be doing the supporting is not a stranger on the first day. The schedule is built around the routine that already works, not around our convenience.
If we cannot take it, we say so promptly and explain why. A referral we cannot cover consistently helps nobody, and a family left waiting on an answer that never arrives is worse off than one given a clear no.
